Overview
Pocket Med List (“the app”) is operated by Chalice Health (“we,” “us”), based in Midlothian, Texas. The app helps you organize medications, set reminders, and share your medication list with the care team members you choose. This policy explains what information we collect, how we use it, and the choices you have.
Information we collect
- Account information — such as your name and email address when you create an account.
- Health information you enter — medications, doses, schedules, dose history, allergies, notes, and optional profile details (such as date of birth, emergency contact, or insurance references) that you choose to add.
- Care team information — the names and email addresses of people you invite, their permission level, and the activity log of changes made to a shared list.
- Device and usage information — basic technical data such as device type, operating system, app version, and diagnostic or crash information used to keep the app working reliably.
How we use information
- To provide the app's features: reminders, refill tracking, safety checks, streaks, and care team sharing.
- To show your medication list to the care team members you personally invite, at the permission level you set.
- To respond to support requests and communicate with you about your account.
- To maintain, secure, troubleshoot, and improve the app.
- To comply with applicable law.
What we don't do
- We do not sell your personal health information.
- We do not make your medication list visible to anyone you haven't invited.
- We do not use your health information to make medical decisions for you — the app is an organizational tool, not medical advice.
Sharing with your care team
Care team sharing only happens at your direction. When you invite someone, you choose whether they can view your list or also edit it. Every change made by a care team member is recorded in the activity log with their name and time. You can pause, resume, or remove a member's access at any time from the app.
Service providers
We use trusted service providers (such as cloud hosting, analytics, and crash-reporting services) to operate the app. These providers process information on our behalf under agreements that limit how they may use it.
Advertising and future changes
The app is currently free to download and does not display third-party advertising. As Pocket Med List grows, Chalice Health may introduce optional paid features, sponsorships, or advertising to support the service. If our data practices change as a result, we will update this policy and notify you within the app before those changes apply to you.
Data security
We use administrative, technical, and physical safeguards designed to protect your information, including encryption in transit. No system can be guaranteed 100% secure, so we encourage you to use a strong password and enable the app's biometric lock.
Data retention and deletion
We keep your information while your account is active. If you delete your account, your data — including medications, dose history, care team connections, and profile information — is permanently deleted within 30 days, except where a longer period is required by law. You can delete your account from inside the app (Profile → Delete My Account) or by following the steps on our account deletion page.
Children
Pocket Med List is not directed to children under 13, and we do not knowingly collect personal information from children under 13. A parent or guardian may manage a child's medications within their own account as a caregiver.
Your choices and rights
- You can view, edit, or delete the information in your account at any time within the app.
- You can remove care team members or leave a care team you've joined.
- You can request a copy of your information, or ask questions about this policy, by emailing support@pocketmedlist.com.
- Depending on where you live, you may have additional privacy rights under local law; we will honor valid requests as required.
Changes to this policy
We may update this policy from time to time. When we do, we'll change the effective date above and, for meaningful changes, notify you in the app or by email.
